Job Title: Advanced Practice Provider
Job Description
We are seeking a dedicated and mission-aligned Advanced Practice Provider to join our integrated care team. The Advanced Practice Provider will provide high-quality, age-appropriate, and specialty-specific care to LLCHC outpatients in coordination with a collaborating physician. The APP will practice within their scope of licensure, certification, and education, and will work alongside a multidisciplinary team to deliver holistic care in alignment with LLCHC’s faith-based mission.
The ideal candidate is a collaborative leader and clinician who thrives in a community health setting, is committed to serving diverse populations, and is passionate about eliminating health disparities through accessible care.
Qualifications, Skills, Education
Required Qualifications
Master's or Doctoral degree in Nursing or Physician Assistant Studies
Board certification in the relevant specialty
2+ years of clinical experience in a healthcare setting
Strong communication and interpersonal skills
Experience with telemedicine services
Current and unrestricted state license to practice as an NP or PA
National certification from recognized certifying bodies (e.g., AANP, ANCC, NCCPA)
Current BLS certification (ACLS and PALS preferred).
Demonstrated commitment to serving diverse, underserved, or vulnerable populations.
Strong interpersonal and communication skills with a team-oriented approach.
Preferred Qualifications
Prior experience in a community health center or similar primary care setting.
Familiarity with EPIC or other EHR systems.
Bilingual (Spanish, Somali, or Arabic) is a plus.
