Job Title: Nurse PRactitioner (NP/PA)

Job Description

This position is responsible for providing efficient, quality health care services, medical screenings, and primary care to the patients of Inner City Health (ICH). The ideal candidate will provide medical care that is culturally appropriate and focused on prevention. This position provides the full scope of primary care services which fall under his/her field of training, including but not limited to diagnosis, treatment, therapeutic procedures, coordination of care, preventive care, and health maintenance to all ages of patients.

The ideal candidate will be positive minded with a passion for serving those communities we serve. The Physician Assistant/Family Nurse Practitioner, while working closely within a diverse patient population, will build strong relationships within the community, delivering personalized medical care in an integrated healthcare environment. In this capacity, they are expected to be familiar with the mission and vision, of the ICH and work collaboratively to achieve them. They must be cognizant of and comply with all policies and procedures, as well as pertinent state and federal regulations.

Essential Duties and Responsibilities

  • Provider shall provide high quality, comprehensive primary care to clinic patients, including education, preventative, diagnostic, and therapeutic procedures within established standards for Provider’s training and experience and within the approved scope of practice.

  • Demonstrates knowledge of the principles of growth and development over the life span and stays current with changes in this area.

  • Perform physical examinations, record and document physical findings within the EMR System, review data along with health history to formulate a definitive diagnosis based on patient's condition.

  • Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and healthcare needs.

  • Prescribes medications to patients, including counseling patients regarding the proper use and contraindications of medications as well as detecting and diagnosing adverse drug reactions. Review and evaluate treatments to determine success.

  • Collaborate with an integrated healthcare team to determine a comprehensive patient healthcare plan.

  • Refers patients to other healthcare practitioners for consultations or specialized health resources for treatment as appropriate.

  • Evaluates medical charts to determine appropriate patient care, follow up and education regarding acute and chronic illnesses.

  • Performs other duties assigned by employer from time to time that are commensurate with professional services, normally and customarily, performed by a physician.

  • Participate in peer review, quality assurance, provider meetings, and other clinical meetings as assigned.

  • Complete accurate and timely documentation for all patient interactions.

  • Provide accurate documentation for appropriate billing of services.

  • Maintain confidentiality of all patient information.

  • Work collaboratively and professionally as a member and leader of a health care team, building and maintaining constructive and cooperative working relationships.

  • Treat patients and coworkers with respect while leading the medical team toward health care excellence.

  • Arrive at work on time and endeavor to deliver timely medical care within the constraints of patient acuity.

Education and/or Experience

  • Graduate of a Master’s level accredited nurse practitioner program or physician assistant program.

  • Board certification.

  • Licensed or eligible for licensure in Colorado

  • Previous experience as a nurse practitioner or physician assistant in a clinical setting is preferred.

Physical And Equipment Requirements

  • Normal work environment is an office in a primary care clinical setting, which involves some physical risks requiring basic safety precautions.

  • This position routinely requires sitting, standing, bending/stooping, operating a computer for extended periods, and lifting or carrying objects up to 25 pounds.

  • Vision abilities required by this job include close vision.

  • This position requires the ability to hear routine conversations and to have the ability to comprehend both oral and written communications
