Overview

We are a community health center providing affordable, bilingual and culturally-sensitive primary care and support services from its three locations in one of urban Pennsylvania’s most medically under-served communities.   Services provided include: primary adult and pediatric care; dental care; prenatal and OB/GYN care; behavioral health consultation services; social services; spiritual care; on-site medication dispensaries; nutrition counseling; comprehensive HIV care; diabetes care; and, a range of new community-based health and wellness initiatives. Our holistic approach to care takes into consideration the physical, spiritual, social and psychological factors that impact a person's overall health status.

Job Title - Director of Quality Assurance and Population Health

Job Description·   

FQHC in Pennsylvania seeks a full-time, board certified Physician / Nurse Practitioner / Physician Assistant / Registered Nurse to serve as the Director for Quality Assurance and Population Health

· The Director for Quality Assurance and Population Health is responsible for the overall administration of Quality Assurance Team and clinical risk management initiatives.

· Lead and coordinate quality improvement measures across all departments of the health center, monitor and improve clinical quality measures and serve as a clinical lead for grant reporting,

·  Lead Population Health overseeing Patient Centered Medical Home, track and improve hospital and emergency room utilization, lead participation in Quality Incentive Programs from payers and Clinical Risk Management programs, and supervise Department of Care Management.

Job Qualifications Education, Skills, and Experience         

·  MD / DO / CRNP / RN or PA-C, eligible for PA licensure for clinical practice,

· MPH preferred,

· 2+ years of clinical experience required,

· Bilingual in English and Spanish, or willingness to participate in overseas immersion language training, required,

· Ability to integrate faith and practice,

· Demonstrated ability to juggle multiple priorities in a fast-paced environment,

· Strong leadership skills and supervisory experience required,

· Excellent written and oral communication skills,

· Strong administrative and clinical skills,

· Experience with grant management, preferred,

· Demonstrated ability to track and analyze data, 

· Bilingual in English and Spanish, or willingness to participate in overseas immersion language training, required,

· Ability to integrate faith and practice
