Job Title - FNP/PA for acute/primary care clinic (part-time)

Job Description·  

  • Provide a scope of primary care services commiserate with experience in coordination wit the collaborative partner, including but not limited to diagnosis, treatment, care coordination, preventive care and health maintenance to patients (2 years of age and above) in both a direct-pay patient setting and in support of a volunteer-based clinic model.

  • The provider will maintain an active collaborative agreement with the collaborative partner, including chart sign-off as mandated by state law.

  • The provider will serve as leader to medical support staff as well as a resource to our volunteer providers and volunteer support staff.

  • The position will work alongside the Clinical Services Manager to ensure quality of care and support of our overall mission.

  • The provider must be available to staff and patients after hours, if needed for consultation.

  • We are seeking a one day a week nurse practitioner (FNP) or physician’s assistant (PA).

  • The provider will need to have a current licensure in good standing and able to obtain licensure in Indiana.

  • Applicants must have well-developed writing, interpersonal and communication sills, as well as be able to use Electronic Medical Records for all charting.

  • The position reports to the Executive Director


Job Qualifications Education, Skills, and Experience

  • Provides continuing, comprehensive health maintenance and acute medical care to individuals and families--to include preventive medicine, behavioral sciences and community health.

  • Obtain health history and perform physical examinations.

  • Diagnose and direct counseling for each patient on a plan for treatment.

  • Prescribe medication in accordance with Indiana statute and professional practice guidelines.

  • Fluent with current standards of care in family medicine, including gynecology, pediatrics, geriatrics, and basic psychiatry.

  • Perform minor surgical procedures commiserate with training and licensure as well as collaborative agreement and CMO relationship.

  • Determines need for consultation and assists in medical care treatment provided at the direction of other specialists.

  • Arrange referral for patients based on need in coordination with the Clinical Services Manager.

  • Review incoming reports (e.g. lab, x-ray, EKG) sign, date and arrange follow-up in a timely manner.

  • Instructs, in conjunction with the Chief Medical Officer, other health care providers and non-medical personnel in a variety of health related topics, including first-aid measures and health care maintenance.

  • May participate in the instruction of students who participate in a clinical rotation at Hope Healthcare Services (the clinic is not currently a rotation site).

  • Assist in staff outreach and off-site recruitment/fund raising efforts as necessary.

  • Comply with the Hope Healthcare Services Infection Control plan which includes personally utilizing universal precautions. Ensures compliance by support staff

  • Participate and ensure compliance with best practice protocols within Hope Healthcare Services as well as all regulatory compliance measures at the federal, state, and local level.

  • Participate in CME, peer review, quality assurance, provider meetings, and other clinical meetings.

  • Assist in updating protocols and principles of practice, as requested.

  • Assist in outside medical and/or public relations functions as necessary.

  • Adhere to and assist with demographic acquisition needs of the clinic and its supporting organizations.
